1. Identification
Once a youth or caregiver expresses an interest in the BYSSI program they will have the option to virtually connect with a Youth Engagement Coordinator to learn more about our after school online services.
At the end of that meeting if the family decides which BYSSI supports would be helpful and BYSSI Digital Intake form will be completed to enroll their children.
2. Referral
BYSSI is contacted by the student’s home-school. Where possible, school administrators are encouraged to participate in the referral process by identifying potential areas of support for the student. The information provided through our BYSSI Digital Intake form will be used to create a unique ‘plan of care’ for the student based on their identified needs and interests.
3. Student Plan-of-Care
The type of supports that would be helpful.
How frequently and the duration of the supports.
The agencies that would be involved in delivering the services/supports.
The goals of the ‘plan of care’ and how BYSSI will track progress.
Lastly, this plan is then brought back to the student and family for approval.
4. Enrolled!
Once the plan of care is finalized and approved, the student is then enrolled and the supports begin!
